Support to live life - Short Breaks
Do you need a rest? Are you looking for an emergency support? Are you going on holiday and your relative you support can’t join you? Or maybe you are sick and need a break from your caring role?
Life Path Trust provides Short Term Support service to family carers and people with a learning disability.
Where is it located?
The service operates from a CQC registered care home with based in a period house in Radford Coventry close to the city centre and amenities.
The service can accommodate up to 10 people with an accessible room on the ground floor providing support to a person using a wheelchair. There is extensive lounge and dining room facilities which provide the space to offer a range of activities and enable people to socialise with others if they choose to.
Can the Short Break take place at home?
We can also offer a short break to people in their own home. If the family carers want a holiday but the person wants to stay in the familiar surroundings of their own home, we can provide staff to support at home.
How long can it be?
It depends on the circumstances and the requirements of the family. It can last a few hours, a few days, a few weeks and sometimes a few months.
How to access this service?
Working in a person centred way we enable the person and the family to plan the break that they want, including the length of stay, activities and how they want to be supported during their stay. We also offer taster stays to enable the person and the family to try the service to see if it meets their needs and requirements.
